FAHPi Executive Director, Doc Mon Seastres, spearheaded the "Rural Care Connect (RCC) Project" which introduces a hybrid geriatric-centred care model for elderly individuals with Type 2 Diabetes Mellitus (T2DM) Hypertension (HTN) and Coronary artery disease in geographically isolated and disadvantaged areas (GIDA) of El Nido, Palawan, Philippines. The model integrates in-person primary care, medical specialists, allied health services, and community networks using digital platforms for virtual consultations, remote monitoring, follow-ups, and health promotion.
